{Slow Cooker} Creamy Chicken Tortilla Soup

1/2 pound boneless, skinless chicken breasts
1 cup reduced-sodium chicken broth
1/2 cup salsa
1 can (14.5 ounces) diced tomatoes
1 teaspoon fresh lime juice
1 teaspoon minced garlic
1 can homemade cream of chicken soup**
1/2 cup chopped bell peppers
1 teaspoon ground cumin
1 cup frozen corn
1 can black beans, rinsed and drained
salt and pepper to taste
sour cream, for topping
cilantro, for topping
